It’s More Convenient
Once you make an appointment, our office will ask you to provide patient registration details such as name and address, responsible party, consent for treatment, etc. For your convenience we have provided many of our forms on our Web site. If you wish, you may download, print and complete these forms prior to your first appointment. This will give you time to prepare ahead and save you wait time. In addition, we find that our patients can provide more complete information when they have access to information from home.
Please remember to bring all insurance cards (both dental and medical) with you to your appointment.
Bring The Forms With You
After you’ve completed the medical history and new patient forms, simply bring them with you on the day of your visit. Alternately, you can also fax them to us at 678-379-5113 prior to your dental appointment — if you prefer.
